Board Certified or Board Eligible Family Practice Physician
Must be bi-lingual in Spanish preferred otherwise Chinese/Mandarin or Farsi
Familiarity with chronic care models and population health management
Mon-Fri - 9-5 - 32-40 hours available
14-20 patients per day
Must be capable of seeing both children and adults
